You must bring all insurance information, (i.e. insurance cards and driver's license) with you on the day of your initial consult visit.

If your insurance requires a referral, please bring it to you appointment. You will not be seen without the referral.

Consent to Participate in Telemedicine Consultation

Patients on CPAP establishing new care with our practice, please bring your complete CPAP device including mask interface and power cord. This allows us to perform a device download to evaluate your treatment efficacy and compliance.
